Individual indicates authorization to United Way of Central Iowa (UWCI) to use and/or share the following of himself/herself and/or child/children for whom he/she is legally responsible in all forms and media for advertising, trade and any other lawful purposes:
- Photographs and/or Video & Audio
- Personal Story
- Use of real names or fictional/partial names at UWCI's discretion
