Implement 5-2-1-0 at your site

5-2-1-0 provides a framework to create healthy environments for kids and to teach kids how to make healthy choices. It is based on a nationally recognized model that provides evidence-based strategies and hands-on support at places where kids spend a lot of time.

173,055 children in central Iowa have demonstrated healthy habits through 5-2-1-0 in schools, child care centers, and health care clinics.

Register your organization to be a 5-2-1-0 site through Healthiest State Initiative and then let us help you incorporate healthy habits into your work. You can also view a map of current registered sites in Iowa:

View the Webinar

This is a presentation applicable to multiple sectors and focuses on environments and attitudes that contribute to the pervasive issue of Weight Stigma and Bias. Participants will learn about the presence of weight stigma; the cycle of bias; the importance of word choice, environment, and practices in minimizing the presence of weight stigma; and how the 5-2-1-0 message applies. The learning may be used in multiple settings.

Child Care and School-Age Settings

We will work with you on how to create a healthy environment and encourage 5-2-1-0 habits with the children you serve. We will start by assessing your current setting and create individualized strategies that work for you. Then we will help connect you to resources and provide assistance to implement these strategies. We can help by:

  • Providing age-appropriate activity ideas
  • Offering promotional materials and curriculum support to encourage health behaviors
  • Creating and distributing health education materials to staff and families
  • Gathering health behavior and habit data
  • And much more!

Health Care

We launched a program for health-care clinics in summer 2017. To participate, clinics are asked to integrate a Healthy Habits questionnaire, place 5-2-1-0 posters in each office, and provide training for staff.
